Library Instruction Request Form

If you are a USFSP faculty member who would like to arrange library instruction for your class, please enter your request on this form. Sessions focus on the specific subject area requested by the faculty member and may also cover topics such as the online catalog, appropriate subject databases, related information sources, evaluation of materials, plagiarism and citation styles.

Library Instruction Classroom Request

This form enables you to request to use the technology enhanced library instruction classroom in POY 218 to instruct students, or supply training to USFSP staff or students.

Library Material Purchase Request

This form enables you to suggest materials for the Library to consider for purchase for addition to the collection. Requests will be evaluated by library faculty based on available funds and USFSP programmatic needs.

Library Reserve Request Form

If you are a USFSP faculty member who would like to place an item on reserve, please enter your request on this form and print it. Submit the form in person to the Access Services desk.

Research Assistance Program Request

This form will send a request to schedule an appointment for specific research needs or for general guidance on how to use the library effectively, and can take from 3-5 days to schedule.

Library Space Reservation Request

This form is used to submit requests to use library spaces. Completion of this form does not imply consent to use the space; you should receive a response within 72 hours upon submission.

USFSP Special Collections and University Archives Request

You can schedule a research consultation, or submit your request for materials online and we will have them ready for view when you come visit us.

Website Feedback

This important form which gathers your important feedback can be used for web site comments or general comments as needed. Depending upon the nature of the comments given, it will be routed to the page content owner, the department head, or the webmaster.

Weekly Challenger Collection Request Form

Students, faculty, staff, and community users interested in learning more about conducting research with The Weekly Challenger collection's resources in Special Collections and Digital USFSP are encouraged to submit their request online and/or schedule a consultation.
